Your Farm Fresh bag will be available Monday afternoon at David Crockett Elementary located at 501 N 36th St, Phoenix, AZ 85008 in the Resource Room. Standard Farm bag- $12, Grande Farm Bag- $20.

Farm Fresh at BUMCP
This project provides free bags of fresh, local produce to patients at Banner University Medical Center – Phoenix who are insured via Medicaid. Produce bags will also be available for purchase for all Banner staff, employees, Residents and Fellows.
Tuesdays from 4:00 p.m. – 6:00 p.m., at the Edwards Building, 1300 N 12th St. Provider Entrance Lobby. Phoenix, AZ 85006
Pre-ordered bags not picked up by 5:15pm will be donated to patients.
